Because of its brief appearance, Ultimate D4C's appearance does not appear to have any major changes, although, Joji describes its ears appear much larger and point and cross towards each other. It appears to have a ring of spikes around its neck.[1]

Ultimate D4C is a more superior version than Funny Valentine's D4C, allowing its user to travel dimensions, though its further capabilities aren't explained in the novel.
History
During his battle with Kars, Whitesnake Ultimate'sWound had begun to evolve. Dio states that he had been down a path similar to this, with Pucci failing him twice, but allowed him to lead up to the moment. Kars prepares to strike him by summoning Ultimate D4C, but fails to do so as Dio's head and his body float into the air.[2]
