Lily and Leo[1] is an unmarked quest in Fallout: New Vegas.

Detailed walkthrough
To start this quest, first reach Jacobstown in the northwest Mojave Wasteland and enter the lodge to speak with Doctor Henry about his nightkin research; this will trigger the side quest Guess Who I Saw Today with the optional objective of talking to Lily Bowen, making her available as a companion. While having Lily as an active companion, the player character has to trigger the unique event of Lily playing a recording of her grandchildren's voices. This requires fulfilling the following conditions (in no particular order):
- Lily must mention "Leo" and one must ask about him ("I have some questions about you" -> "What is the best way to use your skills?" -> "Who is Leo?").
- Lily must suffer a psychotic break during combat; this typically occurs when she has been reduced to 25% or below of her total HP by any means. This includes friendly fire that can occur outside of combat, meaning it can be triggered by purposefully attacking her until she reaches the threshold. Once she calms down and recovers (e.g. "Oh? Did Grandma do all of this?"), one must ask about her condition ("What happened in the fight back there?").
- Lily must be witnessed taking her medicine (e.g. "Medicine Time," "Hush Leo; it's medicine time now" or "Grandma has to take her pills now, Dearie") and one must then ask her about the medicine ("I have some questions about you" -> "Tell me about yourself." -> "What kind of medicine are you using?").
After these conditions are fulfilled, sleep or wait while Lily is the active companion. After this, the interaction will trigger and Lily will play the recording. Due to the way this trigger is scripted, it is best to not sleep or wait after fulfilling Lily's previous three conditions until after midnight (once the in-game clock resets back to 0:00); see below for an explanation of the issue.
After the entire recording has elapsed, one must then ask Lily about the recording, which gets her to reveal that she has only been taking half the recommended dosage of her medicine to avoid losing her memories of her grandchildren. From there, there are three options to complete the quest:
- Recommend that Lily begin taking the complete dosage. If this option is chosen, Lily will no longer suffer from psychotic breaks but her combat effectiveness is reduced - these remain in effect regardless of her mental state. (Melee Weapons skill set to 60)
- Recommend that Lily continue to take half her dosage. If this option is chosen, Lily will continue to suffer psychotic breaks when she is reduced to 25% HP but will now calm herself immediately if one enters stealth and makes her activate her Stealth Boy.
- Recommend that Lily stop taking her medicine altogether. If this option is chosen, Lily will now suffer psychotic breaks when reduced to 50% HP but will gain several permanent stat bonuses - these remain in effect regardless of her mental state. (Melee Weapons skill set to 100)
The choice made will also have an effect on the ending Lily receives.
Notes
- The timer to make Lily play her holotape is set on the first time that the player enters the sleep/wait menu after fulfilling the three conditions. Furthermore, the timer works by taking a float value fRestTimer based on the exact time that that sleep/wait menu is activated, and then playing the holotape once the current time of day becomes greater than the value of fRestTimer. Since Fallout: New Vegas uses a 24-hour clock, this means that a player who chooses to wait for an hour at 23:30 will not be able to hear the recording at least until 23:31 on the following game day. This can make the recording very hard to trigger without knowing exactly what time the timer was set to.
- Thus, once the three dialogue conditions are fulfilled, not entering the sleep or wait menu until after midnight (when the time value resets to 0) is the most reliable way to make sure the recording will play.
- Since it is not possible to sleep or wait to let the time pass, another method to trigger the event is to fast travel to different locations to make the time pass faster.
- Some quest stages can be started through the use of console commands:
- To start the medicine dialogue, enter set "160267".ftimer to -1 and set "160267".bMedicineTime to 1
- To start the holotape dialogue, enter set "160267".fresttimer to 1 and set "160267".bdoonce to 1
- To start the psychotic break dialogue, enter set "13e510".bPsychoticBreak to 1. This won't make Lily enter a psychotic break, but will unlock the dialogue option about it and allow for the quest to proceed normally.
