"Fearless and heavily armored, the Maniac Pointstreak spreads panic through enemy lines, charging headfirst and equipped with only a melee weapon. Found in the Assault Strike Package, the Maniac uses its high defense to get in close while shrugging off attacks. In the right hands, this Pointstreak can be invaluable to your team’s effort. Because of the limited health, it’s important to think tactically when using the Maniac. Check your mini-map and take advantage of the terrain to outflank and surprise enemy players. While you can withstand some direct fire, you won’t last long if you’re engaging enemies head on. Play smart and wreck havoc in your matches."

Maniac (referred to as Juggernaut Maniac by the announcers) is a 10 Pointstreak (9 with Hardline) introduced in Call of Duty: Ghosts. Once obtained from a dropped Care Package, players will receive a Juggernaut-like armor with a normal Combat Knife along with a single Throwing Knife. The Maniac moves at 1.15x normal movement speed in contrast to the Juggernaut and Juggernaut Recon, which both move at 0.8x normal movement speed. The suit comes equipped with a unique set of perks:

As with all Juggernauts in Ghosts, the player's health does not regenerate while operating the suit. However, Maniacs also do not show up as a red diamond on the minimap. Additionally, despite wearing visibly lighter armor and moving significantly faster, the Maniac retains the 92% damage reduction other Juggernauts possess.

  • While moving at 1.15x normal movement speed, Maniacs do not have Agility equipped. This movement speed value is approximately double the benefits of this perk.
  • Attempting to mimic the Maniac's exact class loadout with a normal soldier is not possible without utilizing the Specialist Strike Package, as it requires 14 Class Points (16 if including the class-inherited Hardline), when the maximum given in game is 12 Class Points. This is unique among the Pointstreak Juggernauts in Ghosts, which otherwise effectively obey this limit.
  • A Maniac-like enemy is seen in the mission​ "Homecoming".
  • On the two end sides of the Care Package, the Assault Juggernaut symbol from Call of Duty: Modern Warfare 3 can be seen.
  • Some of the letters on the Maniac's armor are references to Operation: Sand Viper.
  • The first person hand models of the Maniac are reused from first person hand models of Call of Duty: Modern Warfare 3's Juggernaut suit.
