Early Life
The custodian was a young man working as a janitor for an hospital
Attempted thievery
One day, as he was vacuuming the room of an unconscious patient, he checked into the drawer for the patient's belongings. There, he spotted a golden ring, a watch, and a wallet. Unbeknown to the custodian, John Kramer, another patient of the hospital who secretly was a serial killer known as Jigsaw, was able to catch him attempting to steal the belongings by peeping through the room's window and began to imagine him being put into one of his traps, as a way to make him learn a lesson.
Imaginary test
In John's mind, the custodian woke up in a broom closet like room, strapped to a chair that had a wire mechanism trapping the fingers of his right hand and a dial connected to said mechanism on the left armrest. He also had a pair of googles with tubes connected to a vacuum cleaner. The custodian began shouting for help, until he heard the voice of John coming from a speaker. John called out the custodian for his attempt at stealing from the patient and emphasized that "he had his eyes on him", and "didn't like what he saw". John clarified that he sees the custodian's job as a noble one, but needs to do a sacrifice to save both his soul and sight, before revealing that in order to free himself and "live to see another day" he needs to pull the dial into five positions, which would eventually pull his fingers until they break. a sixty seconds timer began ticking on the patient's watch which was strapped to the custodian's wrist and the game began.
The vacuum cleaner began to absorb, prompting the custodian to slowly turn the dial. Despite the pain and fear, he was able to break three of his fingers, but eventually he ran out of time and the vacuum cleaner began absorbing more powerfully, sucking out the custodian's eyes, which navigated the tubes until reaching the vacuum's tank. The imagination segment ended with the custodian moaning in pain.

The custodian then notices John watching him, and hastily puts down the items and hurries out of the room. That is the last we see of the custodian. John tells the custodian 'good choice' as an allusion to what could've happened had he made the wrong choice.
