The leech is a "sensor" for the meathead. When in melee range it bites and poisons the hunter. The meathead near it can sense nearby poisoned hunters.
There are six leeches around a meathead. They investigate the surroundings and move towards the player if they are making too much noise or get too close.

Killing a Leech grants 10 Hunter XP.
