The surface world lies below the floating island. Many of Cave Story's characters come from the surface, for instance, the Doctor, Quote, Curly Brace, and Sue Sakamoto.
Very little is actually known about the surface. Research expeditions have been held to investigate the island above it, as have the reconnaissance efforts of at least two military powers. The surface has been referred to as earth in the Aeon Genesis translation.
